Wristbands & Patient Identification

A report published in September 2013 by the Health & Social Care Information Centre highlighted the following findings:

  • in 2012, 10% of inpatients episodes resulted in errors, half of them preventable
  • of 8 million admissions each year, 850,000 resulted in patient safety incidents, costing £2bn
  • In 2012, 10,000 patients were seriously harmed through mis-identifying, mis-prescribing or wrong surgery
  • Between Feb 2006 and Jan 2007, there were 24,382 reports of patients receiving the wrong care, 2,900 of these being related to incorrect or missing wristbands.

Positive Patient Identification

The ability to identify the patient correctly has to be be the first step towards preventing patient mismatch errors. With effect from October 1st, 2013 all patient wristbands produced in the NHS are required to contain a 2D GS1 DataMatrix barcode containing at least the following patient information:

  • NHS Number
  • First Name & Last Name
  • Date of Birth

This is to comply with the ISB1077 standard issued by the Information Standards Board. (click here to download a copy of this guideline).
